This is a 6/12W offset-pin bulb, catalog number 99-0542, also known in the trade as a 380-type bulb. It shows up across the tail lamp and general electrical equipment figures in the parts books, which tells you it's the double-filament kind used where you need a dim running light and a brighter stop light out of the same bulb holder - classic tail lamp duty.

The offset pin base means the bulb only seats one way in the socket, so you don't have to worry about lining up filaments wrong when you drop it in. It's a small, simple item but the right one matters - get the wrong base pattern and it either won't seat or won't switch between running and brake filament correctly.
