A valve guide seats in the cylinder head and gives the valve stem a true, controlled path to move up and down as it opens and closes. Oversize guides like this .004 version come into play when a head has been reamed out to clean up wear or damage, letting the shop fit a guide with a slightly larger outside dimension to restore a proper interference fit in the head.
