This is a pulling tool used to remove the crank pinion and heavy duty cam gear on Triumph 500 unit twins. Its internal thread screws onto the threaded boss of the gear, and tightening the centre bolt draws the gear off the shaft - the job a clamp-type puller can't grip properly.
