This is the mainshaft assembly for the gearbox, sold complete with its low gear already fitted. The mainshaft is one of the two main shafts inside the gearbox shell, and it's what carries drive through the gearbox to the output side, with the low gear riding on it as part of the gear train. Because this comes through as a low-gear-and-shaft assembly rather than loose parts, it saves you the trouble of sourcing and fitting that gear separately.

The older parts book form of this number is T4158, so if you're working from an early manual or a box of parts pulled off a bike years ago, that's the number you may see stamped or written down instead of 57-4158. Both point to the same physical part.
